Documents Required for Utah Welfare Applications
Before applying for Utah public assistance programs such as SNAP, Medicaid, TANF, or CHIP, you will need to gather several important documents. This guide will help you prepare for a smooth application or renewal process.
Common Documents for All Programs
- Proof of Identity: Driver's license, state ID, passport, or other government-issued ID for all adult household members.
- Proof of Residency: Utility bill, lease agreement, or official mail with your Utah address.
- Social Security Numbers: For all household members who have one (not required for certain immigrants or non-citizens).
- Proof of Citizenship or Immigration Status: Birth certificate, U.S. passport, naturalization certificate, or immigration documents.
SNAP (Food Stamps) Documents
- Proof of Income: Recent pay stubs, self-employment records, Social Security award letters, unemployment, or child support statements.
- Proof of Expenses: Rent or mortgage receipts, utility bills, child care costs, and medical expenses (for seniors/disabled).

Medicaid & CHIP Documents

- Disability or Medical Condition Proof: For certain groups, you may need a doctor's statement or disability determination.
